Direct Referral

You can also refer patients directly to us by providing their information along with any relevant details about their orthotic needs. Simply fax the referral to:

Once we receive the referral, we’ll reach out to the patient to schedule their appointment at our clinic.

ADP Form

Another option is to provide the client with a signed Assistive Devices Program (ADP) form, including the doctor’s billing number and diagnosis. This form ensures coverage for their orthotic requirements. Patients can schedule an appointment using the contact details above and should bring the signed ADP form with them.

Prescription

When making a referral, you can provide your client with a prescription outlining their orthotic needs. This prescription should include details specified by the referring physician. Patients can then schedule an appointment with us by contacting:
